FERPA Release of Information Form | Form for current students to grant access to student record information to a third party. |
Major and Minor Declaration and Changes | Form for students to declare or change majors and minors. |
Major and Minor Program Course Substitutions and Waivers | Form for academic departments to communicate course substitutions and program waivers for majors and minors. |
Individually Designed Majors or Minors | Form for students and mentors to complete to define the motivation and parameters for an individualized major or minor. |
Petition for Exception to Policy | Form to request special consideration to deviate from AC policies and procedures – used for actions such as late add, late drop, late change of grading system, approval for transfer course not typically accepted at AC, course overload, and more. |
Transfer Approval Form | Form for current AC students to request a review of courses and approval that they plan to take to transfer back to AC; Articulates the requirements met for a degree. |
Student Proposal for Directed or Independent Study or Honors Project Form | Form for students to propose an independent study (490 courses), directed study (260, 360, and 460 courses), and departmental Honors projects (491 courses); After this approval has been granted the course number will be created for registration. |

Transcript Request Form | Form to request that an official transcript be sent to a particular institution/organization; Current students make requests in WebHopper and via the National Student Clearinghouse. |

Permission to Work On-Campus | Used by international students to gain permission to work on campus. Must be signed by the potential employer and returned to the Registrar’s Office for review and permission. |
Reinstatement Request Form | Form to initiate the reinstatement process for an F-1 visa when out of status has been reported to SEVIS. |

Special Topics Proposal Form | Form completed each time to offer a departmental 250, 350, or 450 course. Must have approval from the chair/director, dean, and VPAA. |
New Course Proposal Form | Form to request to add a course to the offerings for a department or program; Also used to make a former special topics course a regular course offering and petition changes in Breadth, Writing, Quantitative or Language Competencies. |
Course Changes Request Form | Form to petition the Curriculum Committee for changes to an existing course in the catalog. |
